Are you accepting new patients?
Yes, Dr. Levy is happily accepting new patients.
Does the doctor work with children?
Yes, Dr. Levy is a parent and works with patients of all ages. His patients range in ages from 6 months to 100 years old.
What do I need for my first appointment?
Completed patient registration forms and signed HIPAA release form.
You will need to have your insurance card(s) present and a photo ID (for minors, parent’s photo ID is required).
You will be expected to pay the appropriate co-pay at the time of service.
What do I need to do to pick up my Orthotics?
If you have had Orthotics in the past then all you need to do is stop by the office
during regular business hours to pick them up.
If you have never had Orthotics before then we recommend that you meet with the doctor for a few minutes so he can go over a few basic instructions with you.
